Understanding Rate Types

This is the single most important factor in long-term Medigap cost — more important than the first-year premium.

🟢 Community-Rated

Same premium for everyone, regardless of age. Premium rises only with medical inflation — not because you get older. Best long-term value if you stay enrolled for many years. Example: AARP/UnitedHealthcare.

🟡 Issue-Age

Premium is locked at your enrollment age. Doesn't increase just because you get older — only rises with inflation. Moderate long-term risk — better than attained-age if you enroll young. Examples: Anthem BCBS, Medico.

🔴 Attained-Age

Starts lowest, increases every year as you age. By 75–80, often exceeds community-rated premiums significantly. Highest long-term cost. Examples: Mutual of Omaha, Cigna, Humana, Aetna, Humana.

Rate Trend Assumptions

How premiums grow over time depends on rate type. The 5-year and 10-year totals above use these assumptions:

Rate Type Annual Increase Driver
🟢 Community-Rated ~3%/yr Medical inflation only. No age component.
🟡 Issue-Age ~4%/yr Medical inflation only. Age locked at enrollment.
🔴 Attained-Age ~6%/yr Medical inflation + annual age-based step increases.

* Projections are estimates based on historical NAIC trend data and CMS actuarial assumptions. Actual increases vary by carrier and state.
